Job Description

As a Specialist Care Worker, you will provide advanced and tailored support to residents living with dementia or requiring palliative care. You’ll work closely with families, physicians, and care teams to develop and deliver individualized care plans that prioritize comfort, dignity, and well-being.

Key Responsibilities

• Deliver person-centered care for residents with advanced dementia or those receiving palliative care.
  
  
• Provide emotional, physical, and psychosocial support to residents and their families.
  
  
• Monitor and manage symptoms such as pain, agitation, or anxiety using appropriate strategies.
  
  
• Work collaboratively with healthcare professionals to implement and update specialized care plans.
  
  
• Offer guidance and mentorship to general care staff on best practices in dementia and palliative care.
  
  
• Communicate sensitively and effectively with families, keeping them informed and involved in care decisions.
  
  
• Maintain accurate documentation in line with care standards and regulatory requirements.
  
  
• Ensure a calm, comforting, and respectful environment is upheld at all times.
  
  

What We’re Looking For

• Minimum 2 years of experience in dementia care, palliative care, or a related setting.
  
  
• Certification in dementia care, palliative/end-of-life care, or gerontology (preferred).
  
  
• PSW certificate, RN, or RPN qualification (required).
  
  
• Excellent interpersonal and communication skills.
  
  
• Strong understanding of person-centered care principles.
  
  
• Ability to remain calm and compassionate in emotionally challenging situations.
  
  
• Commitment to ethical, respectful, and culturally sensitive care.
  
  

Training and Certification Requirements

• Registered PSW, RPN, or RN in good standing with relevant provincial authority.
  
  
• Specialized training in dementia care or palliative care (e.g., GPA, LEAP, or equivalent).
  
  
• First Aid & CPR certification (current).
  
  
• Clean background check and vulnerable sector screening.
  
  
• Ongoing commitment to continuing education and professional development.
